How to get patients started:

  • The Pods compatible with the FreeStyle Libre sensors have their own NDC/NRC and require a new prescription. Intro Kit: NDC/NRC: 08508-3000-88 Pods: NDC/NRC: 08508-3000-42.
  • For new-to-Omnipod 5 patients, prescribe through ASPN pharmacy partners to access the newest Pod experience at launch, including the new Algorithm Enhancements (the 100 mg/dL Target Glucose option) and the FreeStyle Libre 3 Plus sensor integration. 
  • For existing Omnipod 5 patients transitioning to Omnipod 5 FreeStyle Libre-compatible Pods, send the prescription to ASPN Pharmacies’ New Jersey location. For support, have them contact Omnipod Customer Care at 1-800-591-3455.

How to Rx
The Omnipod 5 Automated Insulin Delivery System is indicated for use by individuals with type 1 diabetes mellitus in persons 2 years of age and older and type 2 diabetes mellitus in persons 18 years of age and older. The Omnipod 5 System is intended for single patient, home use and requires a prescription. The Omnipod 5 System is compatible with the following U-100 insulins: NovoLog®, Humalog®, Kirsty® and Admelog®.

WARNING: SmartAdjust™ technology should NOT be used by anyone under the age of 2 years old. SmartAdjust™ technology should also NOT be used in people who require less than 5 units of insulin per day, as the safety of the technology has not been evaluated in this population.
