New to Omnipod® 5 System

People on Multiple Daily Injections, traditional tubed pump, or on the Omnipod® Insulin Management System (also known as Omnipod Eros, Omnipod UST400, the Original Omnipod System, or “Omnipod Classic”)

Have your patient complete the Get Started Form.

Once we receive the completed Get Started Form, our team will: 

  • Investigate insurance coverage 

  • Connect with the patient to review options 

  • Support coverage restrictions as required 

  • Request an Omnipod® 5 prescription for a patient to be sent to a pharmacy 

Omnipod® Product Specialists are available to help you and your staff with Prior Authorizations or appeals. Contact our specialists at 1-844-810-5217, Monday-Friday, 8 am to 8 pm EST. 

Once your patients’ Omnipod® 5 System coverage is determined, you will receive a Pod Packet request via fax detailing the next steps. 

Pod Packet is the form that will be faxed to your office.  It is designed to reduce documentation, faxes, and call-backs & reduce the burden for your staff by centralizing multiple processes into one request.

Prescribe the Omnipod® 5 System

You will need to write 2 ERx’s; one for the intro kit and one for the refills. 

The Dexcom G6 CGM requires a separate prescription and is necessary to use Omnipod® 5 in Automated mode.

  • Specialty pharmacies: Advanced Diabetes Supply; Albertsons Specialty Care; Byram Healthcare; Edgepark; Gentry Health Services; Kroger Specialty Pharmacy; Community, a Walgreens Pharmacy 

  • Mail-order pharmacies: CVS Caremark; Express Scripts; Humana; Optum 

  • Cash pay option: Costco Pharmacy

Training on Omnipod 5® System

For new Omnipod® 5 System users: in-person or remote training options will be provided by Omnipod® Certified Clinicians to fit the needs of your practice and your patients.  

Upgrading an Omnipod DASH® user to the Omnipod® 5 System.

Training on Omnipod 5® System

For your existing Omnipod DASH® users on Dexcom G6, we are excited to offer a self-guided eLearning experience that they may complete at their convenience to transition to Omnipod® 5 (without having to wait for in-person training).
