The Omnipod 5 Automated Insulin Delivery System is a single hormone insulin delivery system intended to deliver U-100 insulin subcutaneously for the management of type 1 diabetes in persons aged 2 and older requiring insulin. The Omnipod 5 System is intended for single patient use. The Omnipod 5 System is indicated for use with NovoLog®/NovoRapid®, Humalog® / Liprolog®, Admelog® / nsulin lispro Sanofi®, Trurapi® / Insulin aspart Sanofi®, and Kirsty® U-100 insulin.

WARNING: DO NOT start to use the Omnipod® 5 System or change settings without adequate training and guidance from a healthcare provider. Initiating and adjusting settings incorrectly can result in over-delivery or under-delivery of insulin, which could lead to hypoglycaemia or hyperglycaemia.

**It’s recommended to keep the Controller close by to respond to any alerts or alarms.
†Device components must be removed (and the Pod and CGM sensor should be disposed of) before X-ray, Magnetic Resonance Imaging (MRI), or Computed Tomography (CT) scan (or any similar test or procedure). In addition, the Controller and smartphone should be placed outside of the procedure room. Exposure to X- ray, MRI, or CT, treatment can damage these components. Check with your healthcare provider on Pod removal guidelines.
‡ The Pod has an IP28 rating for up to 7.6 meters (25 feet) for 60 minutes. The Omnipod 5 Controller is not waterproof.
*The Pod Experience Kit is a non-functioning, needle free Pod, without the insulin. The Controller is not included.
